Once you have supplied your information via regular or
e-mail, your chart and a recording by Dr. Atwater on audio tape will be
sent to you via regular mail.

e-mail confirmation ...or along with payment to the regular mailing
address listed below: - Full given name at birth.
- Any name change before the first year.
- Name you presently
use to sign checks
(if you use an initial, declare it as it is
used). - The hour and minute of your birth
- The month,
day and year
- Your place of birth.
If a small city or rural
area, declare larger town nearby (what general area of that region's
map). - Include a paragraph about yourself, and any
concerns you may have.
If you order by
regular mail (and have not paid via PayPal), include a money order
(preferred), or personal check - in U.S. funds only - payable to P.M.H.
